The Community Choices (CC) waiver offers an alternative option to nursing home residence by providing qualified individuals the choice to receive assistive services at home rather than in an institutional setting. The waiver serves the frail elderly (age 65 and older) as well as persons with physical disabilities (ages 18-64) who meet nursing facility level of care criteria.
