What is a Special Needs Plan?

A Special Needs Plan (SNP) is type of Medicare Advantage Plan (MA) that provides benefits to members who have Medicare and Medicaid.

The South Carolina Department of Health and Human Services partners with multiple health plans to administer an optional D-SNP program for members entitled to Medicare and receive Healthy Connections Medicaid.
