Waiver Application for the Healthy Connections – Long Term Care Waiver
The South Carolina Department of Health and Human Services (SCDHHS) gives notice it intends to file a 1915(c) waiver application with the Centers for Medicare and Medicaid Services (CMS) for the Healthy Connections – Long Term Care (HC-LTC) (SC.037) home and community-based services waiver.
The proposed effective date for the HC-LTC waiver is Jul. 1, 2027. The following is a summary of the proposed waiver:
- Consolidates the Community Choices, HIV/AIDS and Mechanical Ventilator Dependent waivers into a single, comprehensive waiver program.
- Creates a more flexible, equitable, and streamlined program that better supports individuals with complex needs statewide by integrating the strongest components of each waiver.
- Includes individuals who meet the nursing facility or hospital level of care and receive case management along with at least one waiver service. Eligible groups include adults age 65 and older, adults age 18–64 with physical disabilities (including those dependent on mechanical ventilation), and individuals diagnosed with HIV/AIDS.
- Further strengthens coordinated, whole‑person care by integrating the consolidated waiver into managed care and enhances the ability of Medicaid Managed Care Plans to support participants across all care settings.
An amendment to the current 1915(b) South Carolina Healthy Connections Long Term Care waiver will be submitted to CMS concurrently.
Current CC, HIV, and VENT waiver participants will not be adversely impacted.
