SCDHHS Announces Year One FFY26 Rural Health Transformation Program Awards

MB#
26-032

As described in the Medicaid bulletin issued April 2, 2026, (MB# 26-009), the South Carolina Department of Health and Human Services (SCDHHS) published funding opportunities available through South Carolina’s Rural Health Transformation Program (RHTP) through four of the state’s RHTP initiatives: Connections to Care, Wellness Within Reach, Leveling Up and Shoring Up to Sustainability.

Today, SCDHHS has announced the organizations selected for funding through the multi-level merit review scoring process. The one‑time investments support sustainable improvements in primary care, maternal and infant health, pediatrics and behavioral health, with a focus on facility modernization, technology upgrades and workforce development.

SCDHHS, designated as the lead agency for this program, received 712 applications by the June 1, 2026, deadline. Awards were determined through a multi‑tier merit review aligned with Centers for Medicare and Medicaid Services requirements.

After reviewing all submitted proposals, SCDHHS has awarded 228 projects for a total investment of $167,299,900.69.
