Healthy Connections Medicaid team members and managed care plans will never ask for money, gift cards or something else of monetary value via text, email or a phone call. If someone contacts you asking for something of monetary value to get or keep Medicaid coverage, please report it by contacting the Medicaid fraud hotline at (888) 364-3224 Monday through Friday from 8:30 a.m. to 5 p.m. or by sending an email to FraudRes@scdhhs.gov.

Changes in Freedom of Choice (FoC) Process and Timeframe for Inpatient Delivery of Medicaid Targeted Case Management (MTCM)

Effective Sept. 1, 2018, the South Carolina Department of Health and Human Services (SCDHHS) will no longer use the SCDHHS Phoenix System or its electronic referral portal, https://phoenix.scdhhs.gov/cltc_referrals/new, for Medicaid targeted case management (MTCM) services or require the Freedom of Choice (FoC) form. 

On and after Sept. 1, 2018, anyone attempting to submit FoC referrals for MTCM through the SCDHHS Phoenix System will be directed to a MTCM provider directory list, scdhhs.gov/mtcmdirectory. The SCDHHS Phoenix System will still be used for Community Long Term Care (CLTC) services.

Please refer to the Sept. 1, 2018 version of the Healthy Connections Targeted Case Management Provider Manual to review these changes. As outlined in 42 CFR 441.18(a)(1), the state assures that the provision of case management services will not restrict an individual's free choice of providers in violation of section 1902(a)(23) of the Social Security Act.

Effective immediately, the timeframe for delivery of MTCM in an inpatient setting for the purposes of discharge planning is no longer than 90 days prior to discharge date.
